Maternal Child Health (MCH) Case Management Program

While you and your baby are enrolled in Firman’s Maternal Child Health Programs, our case managers’ main goal is to support you and all your needs. We know that you are the main caregiver for your baby, their first teacher, and their best advocate and we are here to help you reach all the goals you have for yourself, your baby, your other children, and your family.

Maternal Child Health Family Case Management can help you in many ways...

Firman’s Maternal Child Health are a program's to help pregnant women and infants up to the age of one. While in this program, we are here to assist you in the following areas:

1. Pre– and Post– Natal Care;

2. Scheduling and referrals to healthcare through a Primary Medical Care Physician;

3. Registration and information about KidCare and FamilyCare health insurance;

4. Immunization requirements per-age;

5. The importance and scheduling of Well-Child Visits;

6. Conduct Care Plans, goal-setting, home visitation and face-to-face visits;

7. Provide information about WIC and make appointments;

8. Assistance with Food Stamp Program;

9. Provide referrals and resources to other community organizations who offer assistance;

10. Assistance with information about other Social Service Programs and how to enroll; and much, much more!

• Access to Emergency Fund Program which helps clients in need of assistance with gas, utilities, rent, etc. on an emergency basis (funds are limited)
