| Firman’s Chicago Family Case Management (CFCM) is a program to help pregnant women and infants up to the age of one. While in this program, we are here to assist you in the following areas: |

| 1. Pre– and Post– Natal Care; |
| 2. Scheduling and referrals to healthcare through a Primary Medical Care Physician; |
| 3. Registration and information about KidCare and FamilyCare health insurance; |
| 4. Immunization requirements per-age; |
| 5. The importance and scheduling of Well-Child Visits; |
| 6. Conduct Care Plans, goal-setting, home visitation and face-to-face visits; |
| 7. Provide information about WIC and make appointments; |
| 8. Assistance with Food Stamp Program; |
| 9. Provide referrals and resources to other community organizations who offer assistance; |
| 10. Help with information about other Social Service Programs and how to enroll; and much, much more! |
