| • Previous preterm birth; |

| • Pre-pregnancy weight less than 100 pounds; |

| • More than fourth pregnancy or third child expected within 40 months; |
|
| • HIV or repeated STD infections; |
|
| • Under age of fifteen; |
|
| • Current pregnancy is multi-fetus; |
|
| • Victim of domestic violence; |
|
| • Low educational attainment; |
|
| • Homeless or in temporary housing; |
|
| • Previously incarcerated or under house arrest; or, |
|
| • Previously had or currently has a child in DCFS custody. |
