Family Case Management is a unit of the Maternal Child Health Division. The program helps pregnant women and infants (0-12 months) who have annual incomes below 200% of the poverty level to ensure they are receiving adequate social and health services with dignity.

The Family Case Management's office is centrally located in Worth, “The Friendly Village.” FCM program aims to reduce the infant mortality rate by educating our multicultural clients on how to access different services in their communities. Our bilingual and knowledgeable staff will be more than happy to assist you during and after your pregnancy.

The office is open five-days a week from 9:00 am to 5:30 pm.
